NATIONAL SCHOOL LUNCH PROGRAM/SCHOOL BREAKFAST PROGRAM
Dear Parent/Guardian:
The Bais Yaakov Elementary school offers healthy meals each Monday and Wednesday of full school days to students grades 1st-5th. All meals served meet nutrition standards established by the U.S. Department of Agriculture.
Children in households receiving Food Stamps or Temporary Cash Assistance (TCA) and most foster children can get free meals regardless of your income.
Children in households participating in WIC may be eligible for free or reduced-price meals.
If your total household income is the same or less than the amounts on the Income Chart below, your child(children) qualifies for free or reduced-price meals.
TO GET FREE OR REDUCED-PRICE MEALS FOR YOUR CHILD(CHILDREN), YOU MUST COMPLETE A MEAL BENEFIT APPLICATION AND RETURN IT TO THE SCHOOL. USE ONE HOUSEHOLD MEAL BENEFIT APPLICATION FOR ALL CHILDREN IN YOUR HOUSEHOLD. WE CANNOT APPROVE AN APPLICATION THAT IS NOT COMPLETE, SO BE SURE TO FILL OUT ALL REQUIRED INFORMATION. YOU AND THE CHILDREN IN YOUR HOUSEHOLD DO NOT HAVE TO BE U.S. CITIZENS TO QUALIFY FOR FREE OR REDUCED-PRICE MEALS.
INCOME ELIGIBILITY GUIDELINES
(Effective July 1, 2009 through June 30, 2010)
The Income Eligibility Guidelines (IEGs) for free meals/milk and reduced-price meals are developed and published in the Federal Register by the U.S. Department of Agriculture. The guidelines were obtained by multiplying the federal income poverty guidelines by 130 percent and 185 percent, respectively, and by rounding the result upward to the next whole dollar. Weekly and monthly guidelines were computed by dividing annual income by 52 and 12, respectively, and by rounding upward to the next whole dollar.
HOW TO APPLY:
If your household now gets Food Stamps or TCA, the application must have the names of all children you are applying for and the Food Stamp or TCA case number for each child. The adult household member completing the application must sign the application.
If you are applying for a foster child, the application must have the child's name, the child's "personal use" income, and an adult signature.
If you do not list a Food Stamp or TCA number for children in the household, the application must include all the names of children you are applying for, the names of all household members, the amount of income each person got last month and where it came from, the signature of an adult household member find that adult's Social Security Number or the word "none" if the adult does not have a Social Security Number.
VERIFICATION: Your eligibility may be checked at any time during the school year. School officials may ask you to send papers showing that your child(children) should get free or reduced-price meals.
FAIR HEARING: You may talk to school officials if you do not agree with the school's decision about your child's(children's) meal benefit eligibility or the results of verification. CONFIDENTIALITY: School officials will use the information on the form to determine if your child(children) qualifies for free or reduced-price meals. Also, the name and eligibility status of your child(children) may be: (No other use of this information is permitted.)
Given to local Title I officials for allocation and evaluation purposes.
Used for National Assessment of Educational Progress analyses or other authorized purposes.
Given to other federal and State education or State health programs.
REAPPLICATION: If the child(children) you are applying for don’t qualify now, you may reapply at any time during the school year. If your household size goes up, you lose your job, your income goes down, or your household qualifies for Food Stamps or TCA, the child(children) in your household may qualify for free or reduced-price meals.
